Olympia Pharmacy Announces Voluntary Multi-State Recall

On May 29, 2013 Olympia Pharmacy announced a voluntary multi-state recall of all sterile drug products compounded by the pharmacy due to concerns over quality control procedures impacting sterility assurance. Patients who use the sterile product are at risk for possible fatal infections.

Olympia Pharmacy has not received any reports of illness from use of the sterile products. However the recall is being conducted as a precautionary measure with FDA knowledge.

What Products Were Recalled?

The recall covers all sterile products with a use date of 09/25/2013 or earlier that Olympia Pharmacy provided to medical offices. Olympia will notify customers to return the products to the pharmacy.

Reporting Concerns

Any problems experienced with use of any of the pharmacy’s products may be reported to the FDA’s MedWatch Adverse Event Reporting program either online, by regular mail, or by fax.
