On March 20, 2013 Medprep Consulting, Inc. accounced a voluntary recall of all lots of all compounded products due to mold contamination. The nationwide recall stemmed from a Connecticut based hospital reporting that it had noticed visible particulate contaminants in 50 ml bags of Magnesium Sulfate 2gm in Dextrose 5% in water, 50ML for injection intravenous solution confirmed to be mold. Patients could face fatal infection if administered an intravenous product contminated with mold.

What Should I Do if I have a Recalled Product?

Facilities that recieved any products compounded by Medprep have been informed of the recall and instructed to remove and return the product. Facilities with inquries and concerns may contact the corporation at 732-493-3390.

Reporting Problems

Complaints about the product recall should be directed t0 732-493-3390 between 10 AM and 5 PM on business days.

Report adverse episodes that could be related to the use of the recalled products to FDA’s Medwatch Adverse Event Reporting Program:
